SMS Text Messaging Policy
By providing your mobile telephone number, you consent to receive text messages from Vitale Family Health regarding:
● Appointment reminders
● Appointment confirmations
● Follow-up instructions
● General practice communications
Message frequency varies.
Message and data rates may apply depending on your wireless carrier.
Text messaging should not be used to communicate urgent medical concerns or emergencies. Patients may opt out at any time by replying STOP or by contacting our office. Consent to receive text messages is not a condition of receiving medical care.
Telehealth Disclaimer
Vitale Family Health offers telehealth services when clinically appropriate and permitted under applicable federal and Virginia law.
Telehealth may not be appropriate for every medical condition. Providers may determine that an in-person examination is necessary to ensure safe and effective care.
Patients participating in telehealth visits should:
● Be located in a state where the provider is authorized to practice.
● Use a secure internet connection whenever possible.
● Participate in a private setting to protect confidentiality.
If you are experiencing chest pain, difficulty breathing, severe bleeding, signs of stroke, suicidal thoughts, or any other medical emergency, call 911 or go immediately to the nearest emergency department.
By participating in a telehealth visit, you acknowledge that you understand the benefits and limitations of virtual care and consent to receive services through telehealth when appropriate
